How to put the subscription form on a different website is not working on a standard wordpress blog aside from the one holding the plugin.

1. The subscription from displays correct and seems to function.
2. But when you click on the subscribe button a blank page loads with no error message or any response other

Please anyone be so kind as to point me toward a solution.
