Problem with adding custom checkbox to newsletter

Hello,
I have a problem with setting custom checkbox option to the newsletter form. I’ve created custom form with checkbox option and it’s working fine, but when i want to export my subscribers list to csv. file, the profile1 is empty anyway. Below i’ve added a links to pictures of the problem. I would be very grateful for your help.

That checkbox is connected to the subscription to the list number 1 not the extra profile number 1. And that list must not be private, otherwise that value will be ignored.
