How can I insert newsletter optin into page content?

I have tried using the [newsletter] and [newsletter_form] shortcodes in my pages but it looks ugly.
I want the optin to have a Title and description. The way it looks now is very unappealing to users because they don’t know what the element is for. All they see is 2 fields, name and email. If it said something like “Receive weekly fishing reports” then they would fill in the fields.
Please help.

You should create a custom form and then recall it with [newsletter_form form="1"], for example.
