Form not adding subscribers

Hello,
I am using the same custom form on the home page of my site as I am on the interior pages. The interior pages work fine. The home page form looks like it’s working, but doesn’t actually add any subscribers to the list. It’s the exact same form, so I can’t figure out what’s causing it.
