Custome profile page similar to subscription page?

Hi

I hope you can help.
Can you have a custom profile page formatted to look like the subscription page?
For example, I have a custom profile page as per here – https://dev.tenantstogether.scot/subscribe-test/

I have set up for additional profile fields to display and that is working but I would like to show the same layout as the subscription form, is there a way?
