Problems with suscription

Hi! I installed this plugin in the widget area and when clicked in the submit button gives errors:

I don´t know if I configured something wrong or what’s the problem. Can somebody help me?

Disable the wp_debug in your wp-config.php. I annotated the errors and we’ll work to fix them.
